The
Gender Affirming Care (GAC) Navigator is a community health professional that focuses on the gender-affirming care patient’s needs on an individual basis and at a systems level. The GAC Navigator helps guide the patient through the healthcare system and works to overcome obstacles faced by the patient receiving the services they require. They also work to identify, address, and improve the systems, structures and processes that create barriers to care at the Contact Center, health center, and ecosystem levels. The ultimate goal of the GAC Navigator is to create a path to obtaining
high quality, patient-centered care that is as clear and smooth as possible for transgender, non-binary, and gender diverse patients.
Who You Are:
- Passionate about (and maybe even an advocate for!) reproductive healthcare
- Deeply committed to health equity for transgender, nonbinary, and gender diverse individuals
- Knowledgeable of gender-affirming care, including typical aspects of transition-related medical care, and common barriers to accessing this care
- Talented interpersonal communicator
- Exceptional project management skills; effectively managing progress of goals/strategies whether small or large
- Strong attention to detail, with strong organizational skills
- Ability to prioritize and be proactive
- Experienced or knowledgeable in trauma-informed care and gender-affirming care
What You'll Do: (but not limited to)
- Works with GAC patients to answer questions, provide resources, and identify support needs (including logistical, financial, legal, and community resources) and linking the patient to appropriate resources
- Assists GAC patients and providers to prepare legal letters for gender marker, name change, and other legal documentation
- Facilitates GAC patient referrals to providers outside of the affiliate as directed by clinicians
- Contacts minor GAC patients prior to initial visit to confirm appointment, discuss informed consent requirements, facilitate records request, provide patient education materials and resources, answer questions about cost and financial assistance where applicable, and inform of limitations of care for minors at PPSAT
- Collects, tracks, and reports required program data
- Identifies and addresses gaps and opportunities within existing system processes
- Collaborates with Contact Center and health center staff to provide identify problems and improve processes impacting GAC patient experience
- Acts as contact person for Contact Center for GAC patient scheduling, questions, or patient complaints requiring escalation
- Identifies opportunities for outreach and attends community events to improve visibility of program, strengthen community connections, and build new partnerships
- Identifies, updates, and catalogs community resources and referral resources for GAC patient
Schedule:
Full-Time 40 hours/week
Monday - Friday 9AM-5PM
Hybrid; remote & onsite
Occasional night & weekend work as needed
Potential travel between work sites, not on a regular basis
This position is grant funded for a year, with intention to reapply.

Compensation:
Hiring Range: $18.55 - $21.50
Starting offers are determined by both the candidate's relevant experience and internal equity within the assigned pay grade.
PPSAT's compensation philosophy is based on equity and transparency. Built from data driven compensation studies, our positions are placed within a developed pay grade structure.
