Health Education Coordinator Job at El Dorado Community Health Centers
El Dorado Community Health Centers, with locations in Placerville and Cameron Park, is seeking a full-time Health Education Coordinator. Under the supervision of Compliance and Quality Director, this position is responsible for the coordination and facilitation of lifestyle education, tobacco cessation, and particle pollution education classes for the El Dorado Million Hearts Project, a project dedicated to reducing heart disease and stroke among low-income residents of El Dorado County. The Health Education Coordinator is also responsible for improving health equity among project participants by assessing Social Determinants of Health (SDOH) through surveys and tracking and coordinating with community partners to meet identified needs. This position is part of a multi-disciplinary team dedicated to improving the health and wellness of patients at risk for heart disease and stroke. This position is a 4-year grant funded position through the HRSA Small Healthcare Provider Quality Improvement Program.
Experience & Education
- Master's Degree in Public Health preferred. Alternatively, Bachelor's Degree in Health, Education, Social Services, or a closely related field required.
- Two years experience in administering and coordinating health education or social service delivery programs. (A Master's Degree in one of the above fields may be substituted for one year of the required work experience.)
- Previous grant experience preferred.
- Previous experience working in community health or providing community classes preferred.
- Experience working with electronic health records and tele-video/tele-audio environment preferred.
Other Requirements:
- Must possess and maintain a valid driver's license. Must have a reliable personal vehicle. Must be available at least monthly for evening or weekend classes, meetings, or special events.
- Bilingual in English and Spanish preferred.
