We are currently recruiting for qualified individuals, for the position of Health Program Assistant 1 (PCN 136981) to work within our Community Services Network (CSN) of Greater New Haven. This position will be located at the Connecticut Mental Health Center in New Haven, located at 34 Park Street. This is a full-time, first shift, forty (40) hour per week position. This role's work schedule is from 8:30am to 5:00pm, Monday through Friday.
Discover the Opportunity to Assist:
- With the CSN referral system, including maintaining Qualtrics and PROVIDE referral data systems and producing reports
- With the CSN critical incident reporting system, including maintaining DDAP data system and producing reports
- With the CSN residential utilization management (UM) system, including maintaining UM-related data system and producing reports
- With CSN QA & I systems, assists in activities related to CSN Street Outreach Services referral system, including maintaining SMART Sheets referral data system and producing reports
- With CSN Shelter Plus Care contracts, including producing fiscal reports
- Service delivery monitoring committees and assists with chart audits related to Supported Employment and Supported Education fidelity reviews, program site visits, group home recertification audits, and targeted case management (TCM) reviews
- Attend and assist with Supported Employment and Supported Education Coordinators meetings, Residential Utilization meetings, and Contract Review meeting
The Community Services Network (CSN) is a collaborative of sixteen community-based organizations that provide a broad array of coordinated behavioral health services to people throughout the greater New Haven area. Funded by the Connecticut Department of Mental Health and Addiction Services (DMHAS), the CSN serves more than 5,000 individuals annually, primarily from the towns of New Haven, Hamden, Bethany and Woodbridge.
The CSN is coordinated by the Connecticut Mental Health Center (CMHC), which is Lead Mental Health Authority for the New Haven area. CMHC is a joint endeavor of DMHAS and the Yale University School of Medicine, Department of Psychiatry.

2. A Master's degree in public health, health education, hospital administration or public administration or other closely related field may be substituted for the General Experience.
3. For State Employees one (1) year as a Health Services Worker and a Bachelor's degree or three (3) years as a Health Services Worker may be substituted for the General Experience.
PREFERRED QUALIFICATIONS
- Experience using Excel spreadsheets and web-based data systems
- Experience using initiative, organizational skills, and problem-solving to complete projects on time
- Experience gathering data and interpreting results on public-sector health and behavioral health-related projects and programs
- Experience preparing reports and written communication to both internal and external stakeholders
