TLC currently serves over 700 children and adults and 1,800+ of their family members parents, siblings, and family caregivers. By 2040, our community will have an estimated 38% increase in people living with an intellectual or developmental disability (I/DD).
Minimum Education/ Experience:
Bachelors degree in Psychology, Social Work, Special Education, or related field with one to four years of direct 1:1 experience working with the intellectual developmental disability or behavioral health population. Masters preferred. Certification for NC-SNAP Examiner is required and the course will be provided by TLC. The HCBS Program Manager is responsible for coordinating & scheduling services for enrolled consumers and supervising direct support paraprofessionals. One year of direct support supervisory experience is preferred. Working knowledge of the Innovations Waiver Meets North Carolina requirements to be a "Qualified Intellectual Developmental Disabilities Professional or Qualified Mental Health Professional."
Hours/Days of Work:
40 hours per week (salary, exempt); Monday through Friday 8:30 a.m. - 5:00 p.m.; Schedule must be flexible to meet the needs of the agency and consumers.
Pay Rate: $50k -$60k

Description of Work Performed:
Responsible for screening and admission of consumers to HCBS Program. Responsible for HCBS scheduling system; schedule periodic workers according to authorized hours in treatment plan and needs of consumers/families. Participate in annual ISP Meetings and quarterly Behavioral Review Meetings. Develop ISPs and Short-Term Goals. Actively attend and participate in the treatment team for each enrolled consumer. Ensure service authorizations are in place and do not lapse.
Process HCBS billing. Approve and submit payroll. Assist in annual budget development with the Director of Community Based Programs. Assist in the analysis of monthly financials to ensure the program is meeting or exceeding budget; adjust accordingly if not on target. Prepare and submit annual and monthly program reports to the Director of Community Based Programs. Develop measurable outcome-oriented annual program goals. Recruit, interview, hire, orient, train, supervise and terminate HCBS periodic staff. Conduct regularly scheduled periodic staff meetings, training, and individual staff supervision meetings; Document said meetings and training. Responsible for HCBS care staff 90-day and annual performance evaluations.
Responsible for the day-to-day operation of the HCBS care program. Responsible for participation in and managing HCBS. Responsible for compliance with clinical coverage policies and Medicaid guidelines applicable to the HCBS program; ensure compliance with applicable accreditation standards for the HCBS program. Recommend agency and programmatic policies and procedures to the supervisor and the agency executive team; Implement agency and programmatic policies and procedures. Initiate and maintain relationships with outside provider agencies, stakeholders, Standard Plan Administrators, and LME/MCOs. Attend LME/MCO Provider Meetings; market services at provider meetings and conferences; carry out public relations tasks as needed. Quarterly home visits to assess members well being and health is required.
