POSITION: Independent Living Coordinator

LOCATION: Red Cliff Reservation

WAGE: $20.00 per hour, 20 hours/week

SUPERVISOR: Indian Child Welfare Director

THIS IS A REGULAR PART-TIME NON-EXEMPT POSITION

JOB SUMMARY: The primary goal of the Indian Child Welfare Department is to prevent the breakup of Red Cliff families and to prevent the permanent removal of Tribal Children from the custody of their parents. The person selected for this position is responsible for providing 20 hrs per week working with eligible young men and women with tools and skills needed to transition out of the foster care / kinship care system and live independently as productive members of the community.

DUTIES AND RESPONSIBILITIES:

  • Assist with program inquiries and referrals for youth that may be eligible for Independent Living Services and determine eligibility.
  • Provide direct service to youth who qualify for Independent Living Services. This includes: maintaining consistent visits with each individual case and assisting with facilitation of educational and support groups as needed. Direct service hours should be 50% of the staff’s total hours.
  • Educate youth, Kinship caregiver, Foster caregiver, and/or adoptive families about Independent Living Services.
  • Assess all eligible youth for life skills strengths and needs assessment by completing the Casey Life Skills Assessment with the youth initially, quarterly, and as needed.
  • Develop a plan with the youth that will address the youth’s acquisition of life skills commensurate with developmental level. The plan will include specific information tailored to the youth obtaining: High school diploma and postsecondary education enrollment.
  • Career exploration, volunteering, and shadowing.
  • Safe and stable housing skills.
  • Home management skills.
  • Transportation.
  • Health and medical knowledge.
  • Healthy relationships and marriage.
  • Connection to ethnic culture.
  • Money management and financial self-sufficiency skills.
  • Identify community resources and support systems.
  • Identified life-long connections to supportive caring adults (permanency).
  • Documents including but not limited to birth certificate, social security card, State ID, immigration papers, school records, medical records, etc.
  • Youth’s individualized goals.
  • Identify and apply life-skill development goals and activities that are experiential-based and allow for measurable outcomes.
  • Starting at age 17 ½ collaborate with the youth and their supportive adults to complete the youth’s Independent Living Transition to Discharge (ILTD) plan. This should be a youth -driven process and the plan must be prior to them leaving care and entails specific areas that must be addressed.
  • Interact with Case Managers, other human service agency professionals, community therapists and psychiatrists, community schools and the court system to promote and receive effective collaboration and referrals.
  • Submit client monthly progress reports to team members.
  • Demonstrating as an ideal role model.
  • File compliance and documentation must be adhered to ongoingly.

QUALIFICATIONS:

  • Minimum of high school diploma, HSED or GED required.
  • Associate degree in human services, education, or related field preferred.
  • Two years of experience working with individuals, groups, educational, or community setting assisting others with skill building and/or development.
  • Experience with children who have been placed out of the home in Foster Care, Kinship Care, or adoptive placements preferred.
  • Must have a valid driver’s license, vehicle and appropriate vehicle coverage and maintain driver’s eligibility as a condition of employment. Be eligible to be put on the tribe’s vehicle insurance policy.

PHYSICAL REQUIREMENTS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work in an office-like setting; however, this position does require repetitive movements, lifting, standing, bending, kneeling, reaching and walking.

WORK ENVIRONMENT: The majority of work is a combination of office and a community or home-based environment. Additional work may be performed in a variety of settings to meet client/departmental needs. All tribal buildings are smoke free.

TRAVEL REQUIREMENTS: The ability to travel and attend meetings and trainings; overnight and out of town, both locally & nationally.
