Job Summary:
Under the direction of the Vice President Program Development/Community Relations provide direct counseling to members about availability of entitlement and social service programs. Identify members with unmet social needs and complex medical conditions. Serve as a RCMG liaison to county and community based programs. Assist members to apply for programs that remove barriers to access healthcare.
Essential Job Functions:

- Screen patients in clinics, hospitals and telephonically to determine unmet needs and barriers to accessing healthcare
- Research the availability of quality community services
- Assist in the development of memoranda of cooperation with Community Based Programs
- Assist members who fall into one or more high risk categories to access all relevant entitlement programs and benefits
- Serve as a RCMG liaison to external community based programs
- Work collaboratively with all RCMG operations departments to integrate Managed Care Social Services processes and procedures
- Requires significant judgment independent, independent analysis, critical thinking and knowledge of managed care processes and procedures
- Complete training on departmental procedures, clinical guidelines and Medi-Cal Managed Care program requirements
- Monitor current community standards of care
- Assist members and families with personal and environmental obstacles which contribute to the exacerbation of healthcare conditions
- Assist Complex Case Managers healthcare navigators and hospital Social Worker’s with access to programs needed for successful Discharge Planning
- Work under the direction of clinic lead providers
- Performs related duties consistent with the scope and intent of the position
- Regular attendance
- Travel as required
- Enforces Company policies and safety procedures
- Regularly updates job knowledge by participating in educational opportunities, reading professional publications, maintaining professional networks, and participating in professional organizations
- Minimum of one year experience in counseling or related human services field is required.
- Knowledge of types of financial assistance available to Medi-Cal beneficiaries is required.
- Experience in assessment case management for children, families and individuals with variety of social, psychological, physical and medical problems is preferred.
- Reliable Transportation.
- Excellent communication skills, including both oral and written.

- Must have mid-level skills in Microsoft software (Word, Excel, Power Point).

- Ability to work independently as well as in a team environment.
- Ability to present self in a professional manner and represent the Company image.
- MSW or LCSW is required
- Graduation from an accredited college or university with Bachelor’s degree in human services field such as social work, counseling, psychology is required
- Current California Driver’s License and automobile insurance is required
- The incumbent may travel up to 50% of the time in the greater Sacramento area
