Summary
Park Forest Care Center dedicated to providing top best health care to all of our residents. The RN MDS Coordinator/MDS and Restorative Nurse is accountable for helping achieve the organization’s goals for positive outcomes. This is to be achieved through effective processes, monitoring, training and implementing systems that will lead to appropriate MDS completion and submission, care planning, billing practices, utilization of residents’ benefits and accurate report of quality measure data.
Duties
· Conduct initially and periodically a comprehensive, accurate standardized, reproducible assessment of each resident upon admission, annually (not less frequently than every 366 days) and whenever a significant change is identified in the resident’s status as defined in the RAI User’s Manual
· Assess every resident using the quarterly item set not less frequently than once every 92 days
· Ensure clinically accurate coding and completion of the MDS assessments
· Participate in interdisciplinary meetings, gathering information necessary to the implementation of the care plan and RUGS levels while maintaining an optimal reimbursement level
· Supervise and assist in the preparation, implementation and scheduling of each resident’s MDS and care plan
· Observe and record symptoms and conditions of residents, as required for the assessment process
· Assist Therapies, Activities, Dietary and Social Services with the MDS process
· Monitor timeliness of MDS completion and report to supervisor any outstanding departments or issues
· Transmit completed MDS’ and appropriate tracking forms
· Assist in training and supervision of nursing personnel, if requested by Staff Development.
· Obtain and review various reports monthly, such as CASPER, Quality Measures, SNF QRP and Five Start reports at least monthly to review with the clinical team. Provide insight and analysis of MDS-based Quality Measures
· Review final validation reports and corrections or modifications in response to warnings or errors, as needed
· Participate in care conferences, discharge planning, training, caregiver training, and the provision of resources, as needed
· Complete all other duties as assigned
- Rotating 1 week a month on-call
Responsibilities
· The RN MDS Coordinator is responsible for the overall coordination of each resident’s MDS/RAI/care plan scheduling, implementation completion, and transmission from admission to discharge
· Oversee the baseline care plan and coordinate the completion of the comprehensive care plan according to regulatory requirements. Ensure the Baseline Care Plan Summary is provided to the resident and/or their representative by the completion of the comprehensive care plan
· Complete all care plans, within the specific time frame in accordance with state and federal regulatory guidelines
· Monitor and ensure care plans are adhered to and properly documented
· Work closely with hospital discharge planners and physicians to obtain accurate and complete documentation to support ICD-10-CM diagnosis coding and surgical procedures
· Audit and improve staff education/competency, as needed, to ensure accurate and timely completion of supporting documentation and MDS assessments
· Follow all the policies and procedures set for the in the Policy and Procedure Manuals
· Attend all in-service training sessions
· Maintain positive relations with residents, families and staff
· Maintain confidentiality
· Comply with applicable standards of behavior and conducts including, but not limited to standards of conduct and professional code of ethics
· Wear personal protective clothing, when required
· Complete all training, as assigned, within the specified time frame
· Ensure that the resident’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints are always well-established and maintained
· Assist with nursing duties, when needed
· Be highly involved in determining skilled level of care for Medicare residents and procuring required Medicare specific documentation; be responsible for physician certification of a skilled level of care throughout the Medicare stay; be involved in making eligibility determinations
Job Type: Full-time
Pay: $35.00 - $48.00 per hour
