Job Description:

JOB SUMMARY: Team members are assigned a primary role on the patient access team and may perform admissions, check-in, registration and scheduling functions, as well as address inquiries via telephone, email or fax. The Care Model Process (CMP) guidelines are to be utilized and followed for all workflows.

QUALIFICATIONS:

  • High School graduate or equivalent is required; Preferred Completion of position related 2 year program, such as Medical Administrative Specialist or Medical Office Specialist.
  • Minimum one year experience in a hospital or medical clinic setting or equivalent.
  • Experience in a customer service setting.
  • Excellent verbal and written communication skills.
  • Knowledge of medical terminology
  • Experience with Microsoft products, Word, Excel, Outlook, etc.

ESSENTIAL FUNCTIONS OF THE JOB:

Clinic Scheduling, Registration and Check In

  • Accurately and appropriately schedules patients.
  • Confirm arrival time, appointment time, and any other applicable instructions.
  • Educate patient of any preparation and/or instructions needed prior to procedure.
  • Pre-register patients by verifying demographics and insurance coverage. Create the Hospital Account Record (HAR).
  • Look for existing orders and link them to appointment.
  • Schedule outside physician’s orders received by fax and/or paper
  • Monitor and schedule the Referral WorkQue
  • Place outgoing calls to patients from wait lists, registries, reschedules and other contact requests as needed
  • Accurately and appropriately register and check in patients for their appointments
  • Check in patients for their appointments, complete all needed information and direct them to the appropriate location
  • Provide appropriate forms as designated by clinical staff and/or standard practice
  • Receive and enter appropriately all patient forms dropped off for completion. Determine if the patient needs to be seen to complete the form, and/or have patient complete their portion of the forms. Route forms according to standard practice.
  • Collects and posts patient co-pays/deductibles/balances consistently and accurately.
  • Assists patients with any follow up appointments at check out

Hospital/Outpatient Admissions Clerk and Check In

  • Check in patients for their procedure/appointments and direct them to the appropriate department/clinic/hospital location.
  • Print wrist band and apply to patients per protocol
  • Print patient labels as needed.
  • Perform bedside registration in ER or inpatient unit.
  • Provide appropriate forms as designated by clinical staff and/or standard practice
  • Receive and enter appropriately all patient forms dropped off for completion. Determine if the patient needs to be seen to complete the form, and/or have patient complete their portion of the forms. Route forms according to standard practice.
  • Collects and posts patient co-pays/deductibles/balances consistently and accurately.
  • Assists patients with any follow up appointments at check out.

Care Team Assistant

  • Receive and route incoming and outgoing phone calls and/or e-mails from patients to appropriate pool for clinical response.
  • Open telephone encounters per protocol and complete documentation, utilizing appropriate dot phrase.
  • Monitor Patient Communication folder in InBasket; locate telephone encounters sent from the clinical team, and contact patient as directed.
  • Review Clinic Assistant pool for result letters routed from CMA/RMA/LPN. Print, send letter and “Done” from InBasket.
  • Receive refill requests from patient and/or fax. Document a refill encounter and route appropriately per protocol.
  • Monitor and route incoming RightFax paperwork
  • Complete Pre-Visit Planning and Disease Registries for follow-up and Outreach scheduling

OTHER DUTIES AND RESPONSIBLITIES

  • Greet each visitor/patient with a warm welcome by phone or in person. Smile, make eye contact, and introduce yourself.
  • Answer incoming calls for facility.
  • Maintains confidentiality of all information acquired through employment with the facility, including patient’s medical records and demographic information.
  • Works efficiently with the clinical teams to ensure a smooth flow for the patient and team.
  • Effectively provides back-up for other areas as needed.
  • Completes all other duties as assigned.
