Schedule:
- Full time (80 hours/two-week pay period)
- Salary
- 8 a.m.-4:30 p.m., Monday-Friday, but may be subject to change
- Position will include rotational coverage of Montpelier inpatient weekend and holiday schedules
Qualifications:
- Must be a graduate of CAPTE accredited bachelor, masters or doctorate program
- Must possess a current license to practice as a physical therapist in the state of Ohio
- Must maintain current certification in CPR
Essential Job Duties:
- Evaluates and treats patients who have a variety of diagnoses and presentations
- Provides physical therapy in the hospital's swing bed rehabilitation program, including assist levels up to dependent transfers
- Documents plan of care, treatment and discharge of patients
- Communicates with referring practitioners, case management, nursing, social services and/or insurance providers as appropriate
- Communicates with, delegates to and supervises physical therapy assistants
- Primary work in Montpelier inpatient swing bed rehabilitation, but may assist in other settings/travel between CHWC's facilities in Montpelier, Bryan and Archbold
- Other responsibilities as determined by director
