Schedule:

• Full time (80 hours/two-week pay period)
• Salary
• 8 a.m.-4:30 p.m., Monday-Friday, but may be subject to change
• Position will include rotational coverage of Montpelier inpatient weekend and holiday schedules

Qualifications:

• Must be a graduate of CAPTE accredited bachelor, masters or doctorate program
• Must possess a current license to practice as a physical therapist in the state of Ohio
• Must maintain current certification in CPR

Essential Job Duties:

• Evaluates and treats patients who have a variety of diagnoses and presentations
• Provides physical therapy in the hospital's swing bed rehabilitation program, including assist levels up to dependent transfers
• Documents plan of care, treatment and discharge of patients
• Communicates with referring practitioners, case management, nursing, social services and/or insurance providers as appropriate
• Communicates with, delegates to and supervises physical therapy assistants
• Primary work in Montpelier inpatient swing bed rehabilitation, but may assist in other settings/travel between CHWC's facilities in Montpelier, Bryan and Archbold
• Other responsibilities as determined by director
