Physical Therapists (PT) who are committed to providing the best life and care for our residents.The purpose of this position is to treat residents who have been referred to physical therapy by the resident's physician on written order. Identifying residents by diagnosis or observation or staff referral who may benefit from therapy and requests orders for evaluation and treatment. . Madison is a 60-bed Skilled Nursing Facility that offers both private and semi-private rooms. Our facility offers a wide range of amenities and services specializing in short-stay rehabilitation and long-stay care.
