If the answer is "yes," we want you to know that Tandigm Health has an immediate need for a Population Health Medical Director ready to join our mission to positively disrupt healthcare and bring the benefits of a value-based, population health approach to patient care in the greater Philadelphia region!

In this role, you’ll combine your clinical expertise with your strategic mindset as you partner with our Network Solutions team to drive practice-level improvement across the quadruple aim: quality, cost reduction, patient experience, and provider experience. You’ll develop partnerships with practices throughout the Philly 5-county area* so you can consult on, develop, and guide clinical pathway implementations, referral guidelines, novel approaches to close quality gaps, improvements in the patient and provider experience, new learning collaboratives, and implementation of technologies supporting clinical care and practice transformation. By the end of every project, you’ll have helped move the needle for improving healthcare!

*As the Population Health Medical Director, you’ll regularly meet (virtually and in-person) with practices throughout Bucks, Chester, Delaware, Montgomery, and Philadelphia counties. Additionally, you will be involved with the development of new clinical programs that span across the entire network.

Qualifications for the ideal Population Health Medical Director candidate include...

• Licensed Physician in the State of Pennsylvania (license must be active and free of any limitations or medical board actions/sanctions)
• Current board certification in your field of practice
• Clinical practice experience (10+ years preferred)
• Experience in medical practice operations
• Effective physician management experience, including change management
• Excellent communication skills with patients and practice staff at all levels
• Willingness to devise strategies and roll up your sleeves to help put those strategies into practice!
• Tech friendly

...and these qualifications could make you a high-priority candidate (the more, the better!):

• Management experience in healthcare, insurance, or IPA
• Expertise in quality management and utilization (inpatient and outpatient)
• Experience building effective network and provider relationships
• Demonstrable, effective presentation skills
• Comfortable with Microsoft Office
• Experience in a population health setting
