GENERAL RESPONSIBILITIES*

Provide information and support to people inquiring about housing resources via phone and Transition Projects’ website. Conduct screening and appointment scheduling for the Coordinated Housing Access Team. Assist participants, community partners, and other people who may drop in at the East Portland Office to connect to staff or other resources as needed.

SPECIFIC RESPONSIBILITIES

Respond to all inquiries for Coordinated Access screening and assist with scheduling appointments for housing assessments with CHAT.

  • Conduct monthly follow-up phone calls for program participants placed into housing. Enter data into HMIS.
  • Provide information and support to people inquiring about rent assistance, housing case management, eviction prevention, and other housing related needs.
  • Work closely with housing, retention, and outreach program managers to keep current and accurate information about access to programs.
  • Provide reception, information, and support to people attending appointments or drop in inquiries at the East Portland Office.
  • Assist with general office administration such as preparing correspondence, assembling packets for meetings or training events, filing, copying, and distributing items.
  • Other duties as assigned.

  • Maintain positive relations with participants, volunteers, co-workers, staff from other agencies, agency funders, and the general public.
  • Be tested for TB within 30 days of hire and be retested at least annually thereafter throughout employment.
  • Acquire First Aid/CPR/AED certification within 30 days of hire and be retested biannually thereafter throughout employment.
  • Complete Bloodborne Pathogens and Narcan Training within 30 days of hire and be retested annually thereafter throughout employment.
  • Demonstrate effective communication skills by conveying necessary information accurately, listening effectively and asking questions when clarification is needed.
  • Plan and organize work effectively and ensure its completion.
  • Demonstrate reliability by being present for work as scheduled, arriving and leaving on time and taking breaks in expected time frames.
  • Interact effectively with persons of diverse ethnic backgrounds, religious views, cultural backgrounds, and sexual orientations and treat each individual with respect and dignity.
  • Embrace teamwork and promote a team-oriented environment.
  • Demonstrate initiative.
  • Make decisions that reflect consistency with Transition Projects philosophy, policies, and procedures.
  • Participate in ongoing development and improvement of all program processes and relationships.
  • Attend agency and program staff meetings.

PHYSICAL/MENTAL REQUIREMENTS
The physical and mental demands described here are representative of those that must be met by an employee, with or without accommodation, to successfully perform the essential functions of this job.

  • Hearing and vision adequate for interaction with participants, interaction with staff and interaction with the general public.
  • Hearing adequate for telephone work. Vision adequate for close work.
  • Hand and finger dexterity adequate to operate standard office equipment.
  • Ability to sit at a desk completing paperwork and working on a computer for lengthy periods.
  • Ability to walk, bend, stand, sit, reach, stoop, pull, squat, and climb stairs.
  • Ability to perform CPR for fifteen minutes continuously.

MINIMUM QUALIFICATIONS

The right individual for this position is resourceful and innovative, has a passion for helping others, and is able to deal with difficult situations in positive and constructive ways. This position also requires the following qualifications:
Education/Experience:

  • A Bachelor’s degree in social services or two year’s experience in social services for low income and/or homeless persons.
  • Knowledge of services available to low-income and homeless persons in Multnomah County through Transition Projects and other agencies or the demonstrated ability to obtain such knowledge.
  • Demonstrated ability to work with diverse populations.
  • Ability to follow oral and written instructions.
