Zumbro Valley Health Center is seeking a flexible, team-oriented, and highly motivated Registered Nurse (RN) to join our Psychiatry team. The RN is responsible for providing coordinated, integrated care for clients. The nurse works as a member of a team to provide the highest quality clinical and education services in consultation with physicians, nurse practitioners, case managers, other Zumbro Valley Health Center staff and other community agency staff.

Employment type: Full-time

Hours: Monday-Thursday 7:30-5pm, Friday 8-12pm. No evenings, weekends, or holidays.

Responsibilities

• Complete and document basic health screening and assessments
• Complete accurate and thorough medication reconciliation
• Provide education regarding medications, side effects, and symptom management
• Provide education regarding medical conditions, preventative health measures, and healthy lifestyle interventions
• Complete phone assessment and triage with appropriate referrals, documentation, and follow up
• Utilize patient portals to access external records
• Complete and manage medication refills; communicate with pharmacies and medical providers when indicated
• Monitor, order, and process lab requests and results in a timely and accurate manner
• Complete insurance prior authorization (PA) forms and appeals as needed
• Participate in multidisciplinary treatment plan meetings and integrated case consultations; ensure completion within the required timeframes
• Assist with transitional care/discharge planning for clients admitted to the emergency department, hospital or other treatment settings; coordinate with the client and internal and external treatment teams on recommendations and ensure timely follow-up services
• Complete and follow up on referrals for services
• Complete required documentation, paperwork, and releases of information
• Provide crisis intervention and make referrals to necessary crisis services
• Provide support to psychiatrists, nurse practitioners, and other staff as needed
• Communicate and collaborate effectively with all providers to ensure proper and coordinated care for clients
• Provide cross-coverage for other ZVHC nursing roles as needed
• Perform quality assurance checks

Requirements and Qualifications

• Current MN RN License
• 2 years of experience in a behavioral health setting preferred
• Experience with electronic medical record preferred
• Excellent verbal and written communication skills and ability to collaborate effectively with multidisciplinary team
• Ability to manage workflow and complete necessary tasks independently
• Knowledgeable in the use of Office 365 and telehealth platforms
