As the Rehab Liaison, you will play a critical role in continuing the healing mission of Deaconess.
Job Overview:
This position is responsible for the referral and facilitation of the transfer of patients who meet the criteria for inpatient rehab/SNF and would typically benefit from services provided in a hospital swing bed. This position will work with Case Management to identify and facilitate the transfer of patients for rehab/SNF appropriate care. This includes the communication of accepted patient requirements the assigned DHS Case Manager and others see appropriate.
Required: Certifications/Licenses/Education:
- Bachelor’s or higher from an accredited Nursing School or Rehab Discipline (PT, OT, COTA, PTA).
- A minimum of three (3) to five (5) years of relevant clinical experience.
- Experience in marketing, consumer education, referral development, etc. preferred.

Schedule: Full Time - 80, Day
