The shift is 11am - 11pm during the week and on the weekends it is 12 our night shift (every third weekend).

Provides leadership and support to all patient care units through the facilitation and coordination of patient flow and bed utilization. Establishes positive working relationships with nurse managers, charge nurses and physicians to evaluate current and future requirements for optimal flow. Facilitates external transfers for other hospitals through prompt and accurate triaging requests to the most appropriate clinical service. Actively participates in multidisciplinary teams/committees, supporting patient access and throughput.

Position Responsibilities:
TRANSFER CENTER OPERATIONS: Processes patient transfer requests from external health care facilities. Rapidly triages requests to the appropriate clinical service. Arranges transportation for external transfers as required. Follows up with referring hospitals to ensure complete transmittal of required demographic and clinical data. Actively updates physicians, Nurse Managers and Charge Nurses on status of incoming transfers.
PATIENT PLACEMENT OPERATIONS: Assigns patients to the most appropriate beds based on clinical need and other considerations, including admission criteria, bed availability and staffing. Communicates on a frequent, timely basis with peers, Nurse Managers, Charge Nurses, physicians and other staff to proactively facilitate patient placement and throughput. Reviews admission request from all access points to understand bed demand. Evaluates current and future bed supply to anticipate potential areas of congestion and obstacles to patient throughput. Facilitates resolution when competing demands for bed arise. Appropriately escalates issues to Director.
TEAM LEAD serves as department team lead working in collaboration with the RN Managers and House Supervisors in the overall management and decision making in patient movement in effort to coordinate hospital wide capacity management decisions. Works with Physician Liaisons on capacity management issues. May engage or participate in senior leadership presentations on capacity management issues.
REGULATORY REQUIREMENTS: Complies with EMTALA, Centers for Medicare & Medicaid, TJC, Illinois Department of Public Health and Illinois licensing requirements, as well as with organizational quality improvement programs.
PROFESSIONAL DEVELOPMENT: Engages in and demonstratesself-development in clinical and professional areas. Identifies personal and professional learning needs that relate to organizational goals. Sets specific goals to meet these needs in collaboration with supervisor.
Position Requirements:
Minimum Education:
Required: Bachelors Degree
Specify Degree(s): Nursing
Minimum Experience:
Required: 6-10 years of previous RN experience
Details: Emergency Response, Emergency Triage, Critical Care or Patient Placement experience preferred.
Managerial Experience: 1-2 years
Details: Ability to support and direct team members in a team lead capacity, charge nurse experience
Licensure/Certifications:
Required:Current Registered Nurse License State of Illinois
