1. Demonstrate the ability to engage individuals in a welcoming, hopeful, empathic manner regardless of disability or phase of recovery.

2. Responsible for outreach and engagement activities for individuals and families who receive Easterseals services. Activities may include phone calls, visits, sending letters, and scheduling or rescheduling appointments.

3. Direct services with individuals include outreach by telephone, telehealth, or in person to explore barriers to engagement and appointment attendance, develop solutions, provide information, and answer questions.

4. Assesses and evaluates the needs of individuals and families to develop goals. Demonstrate the ability to identify stage of change and use interventions consistent with stage of treatment. Assists clients in developing strength-based, stage wise treatment matched person-centered plans that are designed to address identified needs. Completes referral forms, clinical and legal documentation necessary to obtain community-based services for individuals served as well as collaboration and communication with other SA/MH community partners.

5. Provide coordination of care and support to population health.

6. Act as a member of team, actively supporting team based care pathways and initiatives

7. Advocates and provides options for supports and services as needed and seeks out and develops community resources to meet the needs of assigned individuals for activities and support services.

8. Educates individuals and provides resources for Self Determination options.

9. Assist individuals with identifying changes in behavior, attitude, feelings and thoughts that are triggers for relapse. Assist in the development of a relapse prevention plan and crisis plan utilizing a variety of supports including personal, community and treatment interventions.

10. Provides case management services for individuals, families and/or guardians for support, rehabilitation and/or crisis intervenion purposes. Assists in commitment procedures when necessary as well as voluntary admissions.

11. Support the use of prescribed medications to treat substance use and psychiatric disorders.

12. Attends team meetings to review problems and plan solutions.

13. Ensures that paperwork is maintained in accordance with Medicaid and Department of Community Health guidelines, as well as Easterseals policies and procedures.

14. Maintains up-to-date knowledge and understanding of Medicaid Provider guidelines and other regulatory standards.

15. Demonstrates knowledge of principles of trauma informed care.

16. Willingness to support and assist consumers with trauma related issues.

• Bachelors or better in Psychology
