Major Responsibilities:
Performs assessment of the client corporation prior to initial coaching session to obtain an overview of the employee work environment. Works with client representatives to determine and promote additional health programs that may be beneficial for the company and its employees.
Assesses client employee needs and assists in identifying solutions for client employee health problem(s) and determines realistic goals with participant. Determines and provides relevant community and healthcare resources that support client employee's goals.
Communicates with service delivery partners, physicians, and other health professionals to provide case coordination and develop a plan of care that facilitates the efficient use of health care resources. Communicates proactively and effectively with participants, peers, and supervisors. Works collaboratively within a team setting to provide positive outcomes for participants, peers, and overall operations.
Assesses, coaches and graduates client employees from care, resulting in appropriately managed caseloads
Attends client company meetings, trainings and other job-specific events to meet client goals and promote services and products.
Performs intake process including client company orientation, establishing client employee wellness appointments and services.
Identifies and facilitates transfers of appropriate cases needing more intensive management to case management.
Performs client employee follow up assessments and makes contact with a member in a disease management program as appropriate.
Acts as a liaison to resolve client company customer service issues, billing issues, etc.
Monitors and prepares utilization reports, outcomes, participation rates, case summaries, etc. for internal and external organizational leadership.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.

Licensure, Registration, and/or Certification Required:
None Required.

Education Required:
Bachelor's Degree in Allied Health or related field.

Experience Required:
Typically requires 5 years of experience in corporate wellness/health coaching or occupational health that includes experiences in wellness and health promotion, corporate consulting, wellness service contracts, marketing/ promotions, establishing new client services, adult teaching, and patient education / counseling.

Knowledge, Skills & Abilities Required:
Exceptional interpersonal, communication (oral and written), and organization skills.
Demonstrated ability to comfortably and professionally present educational seminars to small and large groups.
Demonstrated critical thinking and decision-making skills to work within a high degree of autonomy.
Intermediate computer skills using Microsoft Office Word, Excel, PowerPoint, and electronic mail.

Physical Requirements and Working Conditions:
Exposed to both a normal office environment and a variety of work-site and community environments for program delivery. Work-sites/community environments may contain odors, dust and dirt.
Position requires travel. May be exposed to road and weather hazards.
Operates all equipment necessary to perform the job.
Ability to work flexible hours and shifts.
