The Nurse Practitioner provides professional comprehensive primary medical care to ambulatory CCHS patients in collaboration with health center staff physicians.
Requirements:1. Obtains health history and performs a physical exam to determine a diagnosis and treatment plan in accordance with established standards, procedures, and practices. Requisitions lab tests and x-rays, writes prescriptions, and makes referrals, as appropriate.
2. Provides primary care preventive health services and prenatal and gynecologic services utilizing established protocols.
3. Assesses, treats, and manages specific medical illnesses and conditions according to medical regime, utilizing established protocols.
4. Consults and works collaboratively with physicians and other health care providers to develop effective treatment plans. Provides direction to other personnel and reviews medical records to verify that each patient’s care plan and medical needs are being met.
5. Provides referrals for hospitalization, tests, therapies, or specialty consultations; makes hospital rounds as indicated.
6. Provides follow-up care for patients. Ensures continuity of patient and family care by exchanging information and cooperating with other community health/social service agencies, hospitals, and health care providers.
7. Counsels patients and/or significant others concerning physical and developmental well-being, defining each individual’s needs and problems. Identifies community health resources, and guides patients and/or significant others in their use.
8. Develops and implements plans for teaching and counseling individuals, families, and groups in various settings for the prevention of disease and health maintenance.
9. Attends staff meetings and provider meetings, and maintains involvement with CCHS committees to ensure quality of care for patients; participates in development and/or revisions of specific protocols; participates in peer review, performance evaluations, and the review of improvement data to assess and advance quality improvement measures.
10. May perform other duties as required.
Requirements
POSITION REQUIREMENTS
Education: Graduate of an accredited Nurse Practitioner program; Master’s Degree preferred. Additional training may be required for specialty area.
Experience: Qualified by education, training, or experience working in Women's Health.
Licenses or Certifications: Current licensure or permit as RN in Tennessee. Nurse Practitioner certification or registry eligible required. Additional certification in specialty area may be required. Eligible for Certificate of Fitness (prescriptive privileges) in Tennessee required.
