JOB TITLE: CertifiedNursing Assistant

SUPERVISOR: Director of Nursing

POSITION SCOPE

Ensure quality and safe delivery of personal care services in the home according to the patient’s Plan of Care.

RESPONSIBILITIES:
 Assist in ambulation, exercises, and safe transfers
 Respond appropriately to emergency
 Perform ADL’s on patient with or without assistance of patient
 Provide written or verbal documentation/communication
 Follow written/verbal care plan
 Interact with patient/family/caregiver/representative (if any) in professional manner
 Provide clean, infection free environment for patient
 Travel within area in a timely manner
 Follow directions, either verbal, written or via map
 Submit CNA/NA documentation weekly
 Participates in at least 12 hours of in-service training during each 12 month period
 Performs other duties as assigned.

QUALIFICATIONS:
 Hold a certificate of completion from an approved nurse aide training program or be registered by the Virginia Department of Health Professions as a certified nurse aide.
 Hold a certificate from a professional nursing, practical nursing or nurse aide training program approved by the Virginia Department of Education or the Virginia Community College system or the Virginia Department of Health Professional.
 Hold a certificate of completion from a home health aide-training program approved by the Department of Health prior to July 1, 1984 and been in continuous practice since that time.

KNOWLEDGE, SKILLS AND ABILITIES:
 Ability to read, write and verbally report information to patients, caregivers, representative (if any) and staff.
 Maintenance of confidentiality of patient information is demonstrated according to Agency policy.
 Participates in personal professional growth and development.
 Performs additional related duties that may be assigned by the RN or therapist.
 Knowledge of health, safety, emergency procedures and Agency Emergency Preparedness Plan.

PHYSICAL REQUIREMENTS:
Requires the ability to exert up to 100 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Physical demand requirements are those for Sedentary Work.
