Care Initiatives Hospice is now hiring a Nurse Practitioner (PRN) to join our team.:

The Advanced Registered Nurse Practitioner (ARNP) is responsible to perform requested face-to-face encounters with Care Initiative Hospice patients. The nurse practitioner is responsible to document and report the findings of the face-to-face encounter to the medical director. Functions according to operational policies and procedures to meet patient/family/caregiver needs in compliance with federal, state and local requirements, Care Initiatives Hospice Policies and Procedures, and Care Initiatives Hospice Clinical Team Procedures.

QUALIFICATIONS

Education/Certification/License:

• Advanced Registered Nurse Practitioner (ARNP) education.
• Current advanced level certification by a national professional nursing certifying body as a nurse practitioner.
• Current license with the Iowa Board of Nursing as an ARNP and be in good standing with the Board.
• Current driver’s license and automobile liability insurance.
• CPR certification required before first day of work.
• Home access to the internet.
• Knowledge of care of the terminally ill.

Experience:

• Two years of RN clinical experience is required; Hospice experience is preferred.
• Prefer two years recent clinical ARNP experience in medical/surgical, oncology, home health or public health nursing.

Skills/Abilities:

• Demonstrated tactful, supportive and sensitive approach to patient/family/caregiver needs.
• Ability to exercise independent judgment, plan and organize.
• Ability to effectively communicate both orally and in writing.
• Ability to access businesses and patient homes.

ESSENTIAL FUNCTIONS:

• Completes face-to-face encounters with patients.
• Documents face-to-face patient evaluation.
• Reports finding of face-to-face evaluation to Medical Director.
• Actively participates in interdisciplinary team (IDT) meetings as requested.
• Accurately, completely and timely documents all care provided in patient clinical records in accordance with Care Initiatives Hospice’s documentation requirements.
• Assures confidentiality and safety of clinical records.
• Actively participates in QAPI activities as requested.
