The Lead Service Coordinator is a senior-level position in the Service Coordination department that performs professional service coordination work, by providing daily Targeted Case Management services to eligible children, adults, and their families.

Provides professional service coordination services

  • Link and coordinator services for individuals with Developmental Disabilities
  • Secures Medicaid and non-Medicaid habilitative services
  • Supports individuals served and families to navigate steps in securing appropriate supports and funding
  • Maintains a working knowledge of and locates, refers, arranges, and accesses community resources available for the individual served
  • Manages a caseload of 23-28 individuals, which consists of a wide range of disability areas, age ranges and complexity. If there are SC openings or absences the Lead SC may be carry a larger caseload
  • Secures information such as medical, psychological, and social factors contributing to individual’s situation to assist in completing the Individual Support Plan (ISP); secures supplementary information such as employment, medical and/or school records. The ISP is written by the Service Coordinator in conjunction with the individual, family, guardian, providers, etc. according to CHS and Department of Mental Health – Division of Developmental Disabilities (DMH-DDD) guidelines
  • Research literature, accesses information through the internet, and confers with other Service Coordinators concerning theories and practices in the field of developmental disabilities, behavior assessment, special education, and health care for persons with developmental disabilities
  • Compiles and maintains records on direct contacts, progress notes, monthly summaries, and prepares reports concerning the individual and/or their family and progress toward ISP outcomes
  • Completes case notes in the Set-Works database accurately within three business days of the time the service was provided. Maintaining 78-90+ TCM billable hours per month
  • Facilitates team meetings to discuss progress and possible modifications to services in the ISP; completes addendums as needed
  • Monitors services as required by CHS Policy and DMH-DDD Directives.
  • Serves as a liaison between multiple agencies and individuals served to advocate and ensure that the individual’s needs are being addressed and their rights are protected
  • Attends/participates in relevant meetings of individual served as appropriate
  • Assists Service Coordinators by providing information concerning case management practices, administrative rules and regulations, community resources, vendors, and programs
  • Exercises independent judgement in making decisions related to individual services; receives general supervision from a Service Coordination Supervisor
  • Maintains Missouri Critical Adaptive Behaviors Inventory (MOCABI) Trainer Certification and trains agency staff on MOCABI administration as needed/required, if applicable
  • Administers and/or reviews both formal and informal assessments with individuals served; including, but not limited to, the MOCABI and Vineland for determining eligibility for Medicaid Waiver funding or ongoing eligibility of waiver services. Staff will be trained/certified upon hire, as appropriate
  • Signs off on Requests for Expenditures for processing to the Regional Office
  • Assist with training and orientation of all new Service Coordinators as directed by the SC Supervisor
  • Participate in regular communication with the Service Coordination Supervisor(s), Regional Directors and the Associate Vice President of Service Coordination on trends or concerns identified with the SC Department
  • Assist with internal and external audits as requested; along with generating and analyzing reports
  • Review and approve Level of Care’s (LOC’s) and send the Assessment to the designated recipient

REQUIRED EDUCATION

  • A Bachelor's degree from an accredited college or university a Registered Nurse license.

REQUIRED EXPERIENCE

  • Four or more years of professional experience is required, in working with individuals with intellectual and/or developmental disabilities in a community-based setting. Three of the four years of professional experience required must be met by being employed by the CHS Service Coordination department as a Service Coordinator. Previous TCM or DMH-DD case management experience will be counted as professional experience in St. Louis County.

REQUIRED MISCELLANEOUS QUALIFICATIONS

  • Must provide valid driver’s license.
  • Must provide proof of personal valid vehicle insurance.
  • Must complete a TB Assessment and/or requirements
  • Reliable internet services if telecommuting.

PAY RANGE: $24.89-$26.89/hr.
