Evoke Wellness, operates a collection of free-standing detoxification and residential substance abuse treatment facilities, with locations in Florida, Massachusetts, and an upcoming facility opening in New Jersey.

Evoke Wellness, FL is looking for a Licensed Practical Nurse (LPN). The Nurse is responsible for all Nursing related services for each patient and for linking patients to the physicians and to outside medical resources. They participate in the Interdisciplinary Team Meetings when necessary to inform the clinical team about medical issues and are available for emergency medical situations. The Nurse provides all services under the supervision of the Medical Director. The Nurse is responsible to provide medical & history screens, providing routine nursing care and overseeing the medical and psychiatric management of the patient in coordination with the attending physician. They are also a member of the Treatment Team and provide input into the development of the Master Treatment Plan.

ESSENTIAL DUTIES AND RESPONSIBILITIES : include the following, as well as performance of other work-related duties as assigned:

Completes the following documentation within 24 hours of the patient’s admission to the program:

o Admission Note

o Nursing Assessment

o Pre-Screen Testing

o HRS Consent Form

o Patient Health Examination

o Criteria for Admission Form (if Detox admit)

  • Transcribes Physician’s Orders and Implement all Medical Protocols as ordered
  • Administers medications and monitoring.
  • Confers with and assist the Medical Doctor and the Psychiatrist.
  • Makes appropriate referrals using referral forms
  • Attends and participate in all “in-service” functions
  • Assists with emergency situations and make immediate referrals
  • Provides documentation which reflects services for each patient in accordance with licensure standards
  • Maintains the Confidential nature of all patients and facility activities
  • Maintains required Training & Continuing Education
  • Maintains Standards of Ethical Conduct
  • Meets with patients requesting to see the doctor to determine the urgency of their medical needs and expedite treatment as required
  • Effectively communicates concerns about patient health to the Medical Director and follows up as necessary
  • Under the supervision of the Medical Director, assists in educating patients about health problems and medical care
  • Facilitates medication education for new patients and for all other patients starting new medications
  • Meets with therapists to provide information about patient medical problems
  • Assists the treatment team in incorporating medical issues into treatment planning
  • Accommodate utilization review needs when additional information is necessary to provide further authorization to insurance carriers, this may be accomplished by scheduling clients to be evaluated by the Physician’s Assistant or Medical Director simply by communicating directly with a care manager
  • Scheduling routine laboratory examinations or those ordered by the Medical Director
  • Takes verbal orders from the Medical Director, documents them in the patient record and carries them out accurately and professionally
  • Provides routine nursing care under the supervision of the Medical Director
  • Participate in regular meetings with the Clinical Director to discuss specific patient problems
  • Demonstrates Proficiency in Communication, observation & Written skills
  • Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which are related to work as a member of an addiction treatment team.
  • Participate in regular department meetings and staff meetings.
  • Maintains effective communication between outside medical services and the facility

Requirements :

  • Graduated from a Community College with a degree or diploma of Nursing LPN.
  • A minimum of 1 year experience in a chemical dependency/psychiatric setting, preferably in a detox unit and/or inpatient, partial hospitalization and/or outpatient setting required.
  • Licensed to practice nursing in the state of Florida, maintaining current licensure per state requirements Licensed Practical Nurse (LPN).
  • Maintains current CPR Certified.

Work Environment: Works in a professional environment including professional offices, clinics, hospitals, or in-patient facilities.

Physical Demands: While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop. The employee must occasionally lift and/or move objects up to 25 pounds. INTER-RELATIONSHIPS: Maintains open communication lines with facility staff, attending clinicians and physicians, patients and families. Liaisons with community agencies, vendors, and payor of health services.

POSITION PURPOSE

2. EXPERIENCE: A minimum of 1 years’ experience in a chemical dependency/psychiatric setting, preferably in a detox unit and/or inpatient, partial hospitalization and/or outpatient setting preferred.

3. LICENSURE/CERFICATION: Licensed to practice nursing in the appropriate state; maintaining current licensure per state requirements as a Licensed Nurse Practioner (LPN). Maintains current CPR Cert.

4. SKILLS/ABILITIES:
