Job Title: Lead Nurse Assistant Certified (NAC)

This position includes the following duties:

Central Supply Nurse Assistant

Status: Non-exempt

Primary Focus:

Provides personal individualized care to patients/patients utilizing the Nursing Care Plan in collaboration with other members of the interdisciplinary team to deliver quality care. Provides vital support in all patient care services and be responsible for knowing all of the tools used in the facility regarding nursing units, ER and procedure rooms, as well as sterilizing and packaging instruments. You will also decontaminate clean, process, assemble, store and distribute medical devices and supplies, and keep records associated with sterilization monitors, orders, and inventory.

Required Education and Certification

  • Current NAC licensure in the State of Washington.
  • Current BLS upon hire and duration of employment
  • Minimum of 2 years nurse assistant experience preferred but not required
  • Acute patient care experience preferred but not required

Nursing Assistant Certified Essential Functions Include but Are Not Limited To

Clinical Skills and Patient Care

  • Works collaboratively with peers and team members, using tactful, diplomatic communication techniques in potentially sensitive or emotionally charged situations.
  • Takes responsibility of this position and completes duties with minimal direction from supervisor when appropriate.
  • Assists patients with ADL’s skillfully with consideration for individual needs.
  • Provides personal hygiene measures appropriately.
  • Utilizes correct procedures in order to promote good skin care.
  • Assists patient in using assistive devices in ambulation, dressing, eating, positioning, and allowing opportunity for self-care.
  • Promotes and assists patients’ participation in activities.
  • Provides care outlined utilizing the Nursing Care Plan through utilization of Nursing Orders.
  • Performs competently those technical skills considered basic to the scope of service on the unit.
  • Treats patients, visitors and families with respect and dignity; identifies and addresses psychosocial needs.
  • Assists the patient in the provision of adequate nutrition, including fluid intake and supplements.
  • Follows procedures to aid adequate bladder and bowel elimination/retraining.
  • Adjusts for physical and/or mental limitations of patients/patients.
  • Provides care to avoid the need for physical restraints and when necessary, demonstrates proper application,
  • Documents body functions including vital signs, weight, intake and output measurements and notifies the LN of any abnormalities.
  • Replenishes and orders supplies. Reports all maintenance needs promptly.
  • Supports and promotes unit, department, and facility policies, procedures and goals.
  • Completes assigned cleaning tasks according to established schedule (DME).
  • Provides care for the dying patient and provides postmortem care (may need to assist with transferring).
  • Completes tasks as assigned by the Licensed Nurse within the NAC’s Scope of Practice.
  • Reads, understands, and follows care plans.
  • Performs rehab program to patients/patients as outlined by Therapy Department.
  • Knowledge of Electronic Health Records with complete and accurate documentation of ADL’s, I&O’s, B&B, behaviors, VS, weights or as directed by LN.
  • Able to admit and discharge a patient using the EHR.
  • Knowledge and skills appropriate for all nursing units (North Wing, West Wing, ER, Outpatient Services, Endoscopy and Central Supply) and competent ability to work in all nursing units of OMHC.
  • Able to operate all essential patient care equipment.
  • May need to assist other departments as necessary.
  • Provides training and orientation for new nursing or agency staff as needed.
  • Performs other duties as assigned.

Endoscopy Nursing Assistant Essential Functions Include but Are Not Limited To:

  • Keeps departments directly involved with scope patient care aware of scope day, and number of scheduled patients expected.
  • Acts as the Liaison between the hospital and the providers office for scope procedures: scheduling, cancellations.
  • Ensures the Endoscopy Licensed Nurses and Anesthesia are informed of scheduling and cancelling of scope cases.
  • Assists in providing patient with explanation and verbal reassurance consistently.
  • As needed AND as time allows, may be asked to assist the LN to assist with patient care of pre and post-procedure preparation and:
  • Sets up the procedure rooms and patient rooms for scheduled scope patients.
  • Obtains the physician History and Physical and has available in chart
  • Sets up charts
  • Documents appropriately necessary information per policy.
  • Updates all appropriate scope procedure logs
  • Provides ADL measures appropriately.
  • Assists physicians and nurses in the procedure room. Completes tasks as assigned by the Licensed Nurse within the NAC’s Scope of Practice.
  • Assists in caring out medical and surgical asepsis during treatments and special procedures.
  • Utilizes correct procedures in order to promote good skin care for the patient.
  • Maintains an environment that provides the patient and family with feelings of safety, security, and dignity.
  • Able to adapt to changes with a calm, focused demeanor and response, in a timely manner.

  • Knowledgeable and skilled in operation essential equipment on specific unit. Manages and operates procedure equipment safely and correctly. Keeps up-to-date with equipment, training and skills specific to unit.
  • Supports and promotes unit, department, and facility policies, procedures and goals.
  • Demonstrates knowledge and understanding of patient privacy rights. Maintains confidentiality of all medical, financial, or other sensitive materials and information in printed, electronic or verbal form, which may jeopardize the privacy of patients and/or employees. Accesses and uses the minimum necessary patient identifiable information to perform job responsibilities and duties and only for authorized purposes.
  • Maintains all endoscopy equipment up to necessary standard and contacts and arranges for repair service when needed.
  • Assists in developing and maintaining Policy and Procedures as it relates to scope procedures.
  • Provides training and orientation for new Endoscopy staff as needed.
  • Available for state surveyors when onsite, may be called in on day off.

Central Supply Nursing Assistant Essential Functions Include but Are Not Limited To:

  • Able to adapt to changes with a calm, focused demeanor and response, in a timely manner.
  • Maintains, cleans, and replenishes, inventories and orders supply as needed. Reports all maintenance needs promptly to the appropriate manager.
  • Check outdates on all products and sterile items. Removes and/or re-stock as needed.
  • Autoclave, peel pack items and cloth wrapped items 3-6 months after last sterilization.
  • Perform Autoclave Biosign tests weekly.
  • Completes assigned cleaning tasks according to established Central Supply policy.
  • Knowledgeable and skilled in operation essential equipment specific to Central Supply. Manages and operates equipment safely and correctly. Keeps up-to-date with equipment, training and skills specific to unit.
  • Supports and promotes unit, department, and facility policies, procedures and goals.
  • Maintains all Central Supply equipment up to necessary standard and contacts repair professionals when needed.
  • Develops and reviews Policy and Procedures as it relates to Central Supply annually.
  • Check and re-stock supplies in the ER’s and procedure room as needed.
  • Provides training and orientation for new Central Supply staff as needed.
  • Available for state surveyors when onsite, may be called in on day off.

Schedule

  • Lead Nursing Assistant Certified is a full time position. The primary shift for this position is day shift on the North Wing and 8 hours shifts but in certain circumstances be evening or night depending on the needs of the facility. Central supply will be approximately 4 hours per month and depending on patient case load, the endoscopy schedule is approximately 8 hours per month. Must follow all administrative and nursing policy scheduling requirements.

Physical and Sensory Requirements

  • Must be able to physically, mentally and emotionally meet the needs of the patients throughout the work day.

OMHC REQUIRED RESPONSIBILITIES:

  • Is responsible for becoming knowledgeable and compliant with Department, Hospital, Fire/ Safety/ Disaster, Educational requirements, and Personnel Policies.
  • Follows the Corporate Compliance Program and maintains HIPAA confidentiality standard.
  • Demonstrates support and adherence to the Mission Statement of OMHC.
  • Participates and supports the Quality Improvement program and provides optimum customer service.
  • Mandatory Annual Training per policy completed annually.
  • Complete annual competency and skills
  • Attends mandatory meetings and in-services and other meetings as requested.
  • Checks, reads and responds to district e-mail’s daily when here, at least weekly when not. Is responsible for knowing and understanding the content in the e-mail and follow-up if necessary.

If at any time that I or the facility makes the determination that I can no longer fulfill all requirements within this position, the 8 hours days shift on North will no longer be an option to schedule. If available, you may fill another position within the facility as long as there is an opening and you will be compensated accordingly.

I understand that as a result of my employment, I may be exposed to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals and that the facility will provide to me instruction on how to prevent and control such exposures. I further understand that I may also be exposed to known pathogens and that the facility will make available to me, free of charge, as determined by the Infection Control Nurse, the appropriate vaccinations and immunizations.

I understand that I may be approached with other duties in addition to those described above, and will be given the opportunity to incorporate these changes into my role at OMHC. I also understand that the essential duties of this position as described above may change at any time, according to the needs of the Odessa Memorial Healthcare Center. These changes as well, will be discussed with me and I will be given time to train and incorporate them into my role.

I further understand that my employment is at-will, and thereby understand that my employment may be terminated at-will by the Center or myself.

Check all three ER’s (main ER, procedure room and little ER) making sure that all the rooms are clean and stocked for the day.

  • Check Central Supply and ensure it is stocked, dirty instruments, basins, etc.

a. Wash and put instruments on a towel on counter to dry

b. Once items dry, place items in packages to autoclave along with an autoclave test strip.

  • Check ALL patient rooms on the North Wing to ensure they are clean and ready to admit any patients throughout the day. If any rooms are dirty/used, notify housekeeping.
  • Check clean utility, dirty utility and shower rooms- clean or tidy if needed.

a. Ensure commodes, shower chairs or any other durable medical equipment is clean and ready for patients use.

  • Perform patient specific care on all patients on the North Wing.

a. Check on patients at least every hour for needs.

  • Check and stock the patient rooms with linens (hand towels, washcloth, etc.) and supplies that will be needed throughout the day.
  • Obtain vital signs and weights on patients per provider orders or as directed by the Charge Nurse, document and report to the Charge Nurse.
  • Check with the Nurse for any additional information or Care Plan goals for the patients
  • Check the daily calendar for scheduled outpatients, admits, etc. that might be occurring that day.
  • Responsible to set up rooms for outpatients, admits, or as directed by the Nurse.
  • Re-stock and re-order supplies as needed.
  • Check equipment and report any issues or broken items to maintenance (call or email).
  • Assist in the ER

a. Take patient back and obtain vital signs

b. Place in gown

c. Assist the Nurse in obtaining admission paperwork signature.

d. Toilet patients, take care of personal care needs as requested.

e. Clean ER and re-stock when patient is discharged.

  • When patients are being discharged and after discharge:

a. Assist the patient in gathering up all their personal items

b. Take all linens out of room: bed, closet, etc.

c. Take out garbage.

d. Notify housekeeping

e. After housekeeping has cleaned, restock the room:

  • Gloves
  • O2 canisters
  • Oxygen tubing
  • Anything else to get the room ready for the next patient
  • Check call lights for exposed cords, breakage and notify maintenance if needed.
  • Assist the Charge Nurse with anything as needed within the NAC scope of practice.

Scope Nursing Assistant:

  • Make up all patient charts and have them ready to go by day prior to scheduled scope day.
  • Clean and prepare equipment day prior to scheduled scope day.
  • Call scheduled patients to confirm scope procedure, status of patient and to direct concerns if needed to appropriate person.
  • Check and order all scope supplies needed.
  • Arrange with vendors to send scope equipment for repairs and annual maintenance.
  • Coordinate with front desk/admitting the week prior to scheduled scope day that they have all necessary paperwork- insurance information (these patients many times need preauthorization for the procedure).
  • Chart in appropriate binders and logs ALL necessary information.
  • Follow police and procures for scope.

Central Supply Nursing Assistant:

  • Check and order all supplies as needed.
  • Run weekly Biosign test on autoclave and document in correct binder.
  • Clean shelves in Central Supply frequently
  • Check peel packed items monthly, if needed, re-package and autoclave.
  • Follow policy and procedures for Central Supply.
  • Check for outdates monthly, if outdated, remove and replace/reorder.
Requirements:

Required Education and Certification

  • Current NAC licensure in the State of Washington or if a NAR upon hire, must obtain NAC licensure within 3 months of hire.
  • Current BLS upon hire and duration of employment
