Job Summary:

Responsible for providing emotional support, information, and resources to families of children with disabilities or challenges impacted by COVID-19. Provide guidance and support to community agencies serving the client family population. Maintain accurate records of all contacts with families and professionals.

Essential Functions:

  • Assess the needs of individual who have either tested positive or are suspected to have COVID-19 and their families particular to those with disabilities.
  • Provide case management in linking the families to appropriate community based services and resources for support.
  • Interaction with families can be provided via phone, virtual, or in-person at their home or a community based location.
  • Must be able to establish, develop and maintain collaborative partnerships and networking resources to support the needs of the families.
  • Maintain and manage a case load of approximately twenty (20) families.
  • Keep accurate data and records on all families served.
  • Maintain, manage, and process client records and information in a confidential manner that meets HIPAA and other applicable confidentiality regulations.
  • Position requires data entry, answer phones and various computer activities.
  • Position requires local travel on a daily basis.
  • Keep an accurate mileage log when utilizing own vehicle for home visits, meetings and/or training.
  • EPU facility hours are Monday – Friday; 8:30 AM – 5:00 PM; with some exceptions based on family’s needs. May be required to work evening.
  • Must perform other duties as they are assigned by the Director or Chief Executive Officer.
Requirements:

Knowledge, Skill and Experience:

Education: High school diploma or equivalent with minimum of 12 units in Social Services or other related field or demonstrate knowledge through comparable work experience.

Skills: Must have strong communication skills written and verbal, planning/organizing, information gathering, identifying solutions, summarization, listening comprehension, goal setting, multi-tasking, teamwork, and collaboration, initiative and able to make independent decisions within guidelines, code of ethics.

Experience:

  • Case management and a strong understanding of community resources required.
  • Must be able to work with a diverse population and be a team player.
  • Must have the capacity to engage the family and yet maintain the boundaries of a helping relationship.
  • Good problem solving and communication skills and are able to work with families to empower them to engage in problem solving.
  • Must hold valid California Driver's license and good driving record with no serious violations for the previous two-year period and be able to provide proof of current automobile liability insurance.
  • Must have reliable transportation in order to visit families in their homes.
  • Bi-lingual in Spanish is a plus.
  • Computer literate, knowledge of Microsoft Office products.

Physical Demands:

Typical Working Condition: Staff may be required to visit families either at their home or an agreed upon community based location. EPU does not have control over condition of all the environments staff may be exposed.

Equipment Used: Personal vehicle for local travel, computers, printers, telephones, and fax machines.

Essential Physical Tasks: Working with families may require the ability to sit, reach, twist, turn, kneel, stand, stoop, crawl, lift, carry, push and pull.
