This position will offer their services to Home Care by Cypress at Home which provides non-Medicare, private duty home care support.

Our RN’s provide comprehensive nursing care that assists clients to achieve optimum health and quality of life in situations of chronic disease, acute illness, injury or through the process of dying.

WHAT YOU WILL BE DOING:

• Responsible for providing nursing care, per Cypress at Home protocols, through all phases of care taking into consideration age and developmental needs.
• Collaborate and support plan of care as determined by the physician and healthcare team; treat patients accordingly as approved by the physician.
• Perform on-going periodic system assessment and data collection in a systematic manner, focusing on: psychological, psychosocial, cultural spiritual and cognitive status; nutrition; pain; patient/family education; family involvement and patient advocacy; patient and patient’s equipment needs.
• Facilitates timely discharge based on goals and in accordance with client policies and procedures.
• Educates and instructs the patient, patient’s family and other staff as required.
• Counsels the patient and family in meeting nursing and related needs.
• Supervises and teaches LPN’s and HHA’s as required for patient care according to law and regulations.
• Documents all care, intervention and care coordination per law, regulation, and agency policy.
• Assists DON with education, in-services, other duties as assigned.
• Preparation and/or administration of medications including preparation of weekly or bi-weekly medication pre-fill packs.
• Administration of both subcutaneous and intramuscular injections.
• Medication monitoring and support for clients with acute or chronic health conditions.
• Minor wound assessment and dressing changes.
• Partner with Resident Services Coordinator, Director of Resident Life, and Wellness Nurses to coordinate transition moves between levels of care and share information with families about health status of resident, in conjunction with HIPAA regulations.

WHAT WE ARE LOOKING FOR:

• Graduate of an accredited nursing school in the state Florida.
• Valid Florida RN license required.
• Current CPR certification.
• Must have valid FL driver’s license and access to personal vehicle.
• Minimum 1 year experience in a home health agency or hospital setting preferred.
